Requisite Information and documents:

1. Notarized & legalized Power of Attorney from the applicant;
2. Notarized copy of Protection Title(s);
3. Description and photos of genuine goods;
4. Description of genuine and possible imitation/counterfeit goods (if available);
5. Photos of genuine and possible imitation/counterfeit goods (if available);
6. List of legal goods importer(s) and/or exporter(s)
7. Mode of import and/or export
8. Information of forecast on when and where related shipment(s) going to be done Customs procedure
9. The IP assessment conclusion issued by the IP assessment authority (if available).
